retinoic acid is one of the derivatives of vitamin A.it is used for treatment of dermatitis, but it has different teratogenic effects on developing organs depending on the different stages of embryonic life.neural tube is made of two different parts:primary neural tube originated from embryo germinal layer and and secondary neural tube originated from tail bud.the persent study was designed to compare the effects of different doses of retinoic acid on primary and secondary neural tube development in the stages of 11-12 humburger & hamilton.in order to do this,254 white leghorn eggs were divided to six groups for futher direct studies.four of the groups were injection with either of:0.3,0.5,0.7, and 1 microgram solution of retinoic acid in 2 microliter of dimethyl sulfoxide, the fifth group was injected with only dimethylsulfoxide, and no injection was made to the last group(sham).forty eight hours later embryos were removed and studied both microscopically and macroscopically.the results showed that, retinoic acid with the dose of 0.5 microgram caused the primary neural tube to be left open and produced accessory neural tube with comparison to the control group(p<0.05).this study suggests that one of the teratogenic effects of retinoic acid in chick embryo is abnormality in the nervous system presented as open primary neural tube or producation of accessory neural tube.

oral lichen planus is diagnosed according to clinical and histopathological characteristics.some times all of the classical histopathological features can not be seen.in some articles the presence of mast cells have been noted below the epithelium of oral lichen planus patients.the following study was done to evaluate the number of mast cells in the histopathological sections of 20 oral lichen planus patients.all of the approved histopathological samples of oral lichen planus were stained with giemsa stain and their mast cells were counted.the mean value and standard deviation of the number of the mast cells in oral lichen planus patients were 47.75+_38.04 in the test and 2.4+_3.57 in the control group respectively.these values were statistically significant(p<0.001).special staining for the mast cells of oral lichen planus sections may be considered as another histopathological criteria for the diagnosis of oral lichen planus.

A limited number of cutaneous leishmaniasis cases has been reported in Kashan region in the past, however during 1995 there was an unexpected outbreak of the disease in the city.in order to determine the prevalence rate of the infection in inhabitants of this area a total of 4856 individuals in ten villages were examined.the individuals infected previously and new cases were identified by clinical examination and parasitological laboratory tests.the results of this study showed that the incidence rate was 14.6%, and 1.5% of the cases had scar.the rate of infection among individuals under 10 years of age was 27.5%. 45.2% of the previously infected cases had more than one scar, and the maximum number of scars observed was 11. the prevelence rate of active sore among the residents in the region was 13.1%.the highest prevalence rate was seen at the age group of 7 to 13(47.6%).53.2% of the individuals had more than one sore and the most common location of sore was on hands, face and feet, respectively.in this study,no relation between sex and infection was found for the new cases of the disease.the incidence rate in the region was 61.35 per thousand.in another study, 2347 male and female students from elementary, secondary and high schools from the same region were examined.the prevalence rate of active sore for these groups was 15%.the highest rate of sore was found among the 10 to 11 year old students.in this study we also found a significant relation between sex and active sore(p<0.05).the history of cutaneous leishmaniasis and its prevalence rate in this region is a clear indication of its presence in the area, therefore, futher epidemiological investigations of this infectious disease in necessary.

Assessment of surgical approaches of supracondylar fracture of elbow in children
H

the aim of this study was to assess different surgical approaches of supracondylar fracture in 31 children.the surgical approches consisted of anteromedial (16 cases),posterior (13 cases) and anterolateral (2 cases). the rate of persistent complications was higher in the posterior approach.the most common complication eas soft tissue atrophy in posterior of elbow (69%) followed by limitation of the range of motion (47%). two cases of cubitus varus and a case of supination was also seen in the posterior approach.in 16 cases of anteromedial approach,14 cases (88%) had excellent results with no complications, however in one case there was a slight limitation of extension and cubitus varus was seen in another case.in the two cases which were operate through anterolateral approach no complication was observed.regarding the findings of this study,it seems that complications of posterior approach are higher than the other approaches studied in this report.therefore because of posteromedial displacement and the tearing of soft tissue, the anterolateral and anteromedial approaches are recommended.

Visceral leishmaniasis in Iran and the role of serological tests in the diagnosis and epidemiological studies
GH.H
Edrisiyan
Kale- azar is known as an endemic doses in some areas of Cars province ( in South) and Ardabil province ( in North- west) of Iran. In other provinces the disease has been reported, mostly, in sporadic from. However since 1949 when the first case of kale- azar was reported in Mazandaran province, to the end of 1993 more than 4300 cases had been diagnosed in at least 113 cities and districts. Kale-azar in Iran is of Mediterranean type and the causative agent is leishmania infantum. Phlebotomus major, Ph. Kandelakii and pH. Perfiliewi that have been found infected with promastigotes in the endemic areas are most probably the main vectors of the disease. Dog is the main reservoir in the endemic areas. Jackals and foxes have also been infected with leishmania in the endemic areas as well as in some other areas of Iran where kale- azar cases have been found in sporadic from. ℅ 15 to 20℅ of the dogs in Meshkinshahr and Germi from the province of Ardabil have been sero- positive also parasitic examinations in most of these cases reveals amastigotes in spleen and liver of these dogs. Recently, gray hamster has been found viscerally infected with leishmania in the Meshkinshahr, kala, azar endemic areas. The disease occurs mostly in children. In the endemic areas of Ardabil and Fats provinces 93.4℅ and 99.3℅ of the cases were found in children of <_12 years old and the highest number of cases were 53.3℅ and 54.4℅ in age group of 1_2 years, respectively . Male or female ratio in Ardabil and Fats provinces were 1.34 and 1.37, respectively. However in serological surveys of kale-azar, sero-positive rates(SPR) in both direct agglutination ( DAT) and indirect fluorescent antibody ( IFAT) tests were higher in females than males. Serological tests such as IFAT, ELISA and recently DAT have been applied in sero-diagnosis and sero-epidemiological surveys of visceral leishmaniasis( VL) in man and animal reservoirs in Iran with satisfactory results. In fact the rapid increase of the incidence of VL in Iran, during recent years, particularly, in the Kala- azar endemic areas is mainly due to increase in application of such serological techniques which in comparison to parasitological methods are more sensitive and quite specific and practical. The DAT among selorogical tests applied in Iran is a simple as well as a reliable test. In the suspected Kala- azar pediatric patients from the endemic areas , a positive DAT result along with clinical symptoms could be considered as a good indication for specific treatment of the disease.
